Responsibilities
Conduct comprehensive assessments to identify strengths, needs, preferences, and goals
Develop and update individualized Life Plans in alignment with OPWDD regulations and person-centered planning principles
Coordinate medical, behavioral health, educational, vocational, and community services
Facilitate interdisciplinary team meetings and ensure all required documentation is accurate and timely
Provide ongoing advocacy to ensure individuals receive appropriate services and supports
Communicate regularly with individuals and families—both in English and Spanish—to ensure understanding of services, rights, and available resources
Assist clients in navigating OPWDD eligibility, benefits, and community programs
Track service delivery, follow up on gaps in care, and ensure Life Plan goals are being addressed
Maintain detailed, timely documentation in electronic health record systems according to OPWDD and agency standards
Participate in audits, reviews, and quality assurance initiatives
Build and maintain relationships with provider agencies, schools, healthcare providers, and community organizations
Work collaboratively with internal teams to improve service coordination and client outcomes
Attend trainings, workshops, and professional development related to OPWDD services and cultural competency
Qualification
Required
Bachelor's degree in Social Work, Psychology, Human Services, or related field (required)
Bilingual fluency in English and Spanish required (oral and written)
Preferred
Prior experience working with the OPWDD population or individuals with developmental disabilities strongly preferred
