Responsibilities
Provide comprehensive, patient-centered primary care to a panel of approximately 450 adult patients
Conduct routine wellness visits, chronic disease management, and same-day sick appointments
Maintain accurate and timely documentation in the electronic medical record (EMR)
Collaborate with in-house care teams including health coaches, nurses, and administrative staff to deliver coordinated care
Participate in preventive care initiatives and ensure adherence to evidence-based clinical guidelines
Develop personalized care plans and monitor patient progress toward health goals
Qualification
Required
MD or DO from an accredited U.S. (stateside) medical school
Board Certification in Internal Medicine or Family Medicine
Active and unrestricted South Carolina medical license
5 years of outpatient primary care experience post-residency
