LPN Residential Rehabilitation Treatment Program
The Marion VA Heal Care System is seeking a full-time Licensed Practical Nurse (LPN) within Residential Rehabilitation Treatment Program (RRTP), Nursing Service located in Marion, IL. The RRTP LPN demonstrates knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.

Responsibilities
Performs vital signs measurement, recording, and monitoring; performs Alco-sensor measurement (Breath Alcohol Tester), monitors and records intake and output {l&O) and patients' weights as ordered; monitors and records neurological checks; Recognizes problems, and takes appropriate actions. Reports significant changes or findings to the team leader or charge nurse
Collects, labels, and sends specimens (sputum, urine, feces, etc.) according to hospital policy and procedure as assigned
Recognizes physical and behavioral changes in patients and reports observations to the team leader/charge nurse
Possesses knowledge of and assists with treatments, diagnostic procedures, therapeutic groups, or other activities as assigned utilizing concepts of whole health and the recovery model
Evaluates and documents physical, mental, and behavioral status of new patients, transfers to the assigned unit, and patients who are post-op or post-procedure. Reports findings to the team leader/charge nurse
Participates in data collection for quality improvement studies; assists in formulating and implementing quality improvement plans on the unit. Participates in GEMBA huddles and engages in process improvement activities that increase safety, quality, and efficiency
Cleans and uses reusable medical equipment according to manufacturer's instructions
Prepares and administers prescribed medications accurately and on time. Observes and records, response to medication administered. Reports any noted change in patient's condition to RN/NOD or MD/DO. Observes and records responses to PRN medications. Reports as appropriate. Provides patient education about prescribed medications. Recognizes when a medication error has been committed, reports immediately to team leader/charge nurse, and initiates incident report. Administers controlled substances. Counts controlled substances
Recognizes emergency situations (i.e., choking, bleeding, seizures, cardiac or respiratory arrest, suicidal threat/attempt, behavioral disturbances, opioid overdose, intoxication, etc), initiates appropriate responses, and participates in basic emergency measures
Qualification
Required
U.S. Citizenship
Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth of the United States, or District of Columbia
Proficient in written and spoken English
Must pass pre-employment physical examination
Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP)
Must complete all application requirements detailed in the 'Required Documents' section of this announcement
Preferred
3 years LPN with 1 year in Behavioral Health or Substance abuse
