This role is responsible for the design, build, testing, implementation, and ongoing support of Epic’s Healthy Planet module, focusing on improving patient outcomes and meeting regulatory requirements through effective population health management and care coordination.

Responsibilities
Configure and maintain Epic Healthy Planet workflows, registries, and reporting tools
Develop and optimize dashboards, care gap alerts, and population health metrics
Collaborate with clinical and operational teams to ensure accurate data integration from multiple sources
Validate data integrity and troubleshoot issues related to registries and reporting
Conduct unit and integrated testing for new features and upgrades
Support go-live activities and provide post-implementation support
Work closely with clinicians, care managers, and IT teams to align system functionality with organizational goals
Provide training and documentation for end-users
Qualification
Required
Epic Certified Healthy Planet Analyst
Experience with Epic Healthy Planet module
Design, build, testing, implementation, and ongoing support of Epic's Healthy Planet module
Focus on population health management, care coordination, and analytics
Configure and maintain Epic Healthy Planet workflows, registries, and reporting tools
Develop and optimize dashboards, care gap alerts, and population health metrics
Collaborate with clinical and operational teams for data integration
Validate data integrity and troubleshoot issues related to registries and reporting
Conduct unit and integrated testing for new features and upgrades
Support go-live activities and provide post-implementation support
