Case Manager - Client Service Center
DuPagePads is currently seeking qualified candidates to join our team as a case manager, working with people who are experiencing homelessness for our Client Service programs. As a case manager, you will be responsible for intake, assessment, case management, and referral services to program participants who use the Interim Housing Center shelter/Empowerment Center supportive services, and for providing advocacy and education on behalf of participants.

Responsibilities
Operates within the DuPagePads Core Values, which supports providing care in accordance with the Trauma Informed Policy and Practices of the Agency
Screen and assess participants who use the Interim Housing and Client Services Programs
Provide individual case management to participants to establish goals and objectives
Work with participants at the Empowerment Center, shelter, and in the community, as necessary
Work with participants to develop individual transition and employment plan
Provide appropriate referrals to community services
Work with participants to ensure they have and maintain all mainstream benefits they are entitled to, establish a primary care physician and relevant specialists, and help resolve barriers to access (e.g. obtains referral approvals, files appeals, manages other documentation to ensure continuity of care, etc.)
Provides individual support and health education to reach healthcare goals. Utilizes psychoeducational and other interventions (i.e. Motivational Interviewing, Harm Reduction, etc.) to teach self-care, goal achievement, lifestyle, nutrition, and disease self-management
Prepare and maintain accurate confidential case files, contractual compliance and paperwork, and status reports
Provide and/or ensure support and enrichment programs to assist participants, including facilitation of groups at the Empowerment Center, as needed
Advocate for increased community education, relationships, and solutions to benefit our client population
Participate in all team meetings, weekly supervision meeting, and monthly department meetings
Attend community meetings to network and develop resources for program participants
Other duties as assigned by supervisor
Qualification
Required
Bachelor's degree in Social Work or related field
Experience in case management
Preferred
Knowledge of issues related to homelessness, poverty, substance use, and mental health preferred
Experience in issues related to homelessness, poverty, and behavioral health preferred
