Clinical Pharmacy Specialist Intern

Responsibilities
Deliver pharmaceutical care services through direct, innovative interactions with patients and healthcare providers
Conduct both face-to-face and virtual consultations to manage chronic disease states with cutting-edge medication management
Utilize advanced techniques to obtain and analyze comprehensive medication histories for optimized patient care
Reconcile patient medications during ambulatory care visits, ensuring safety and efficacy
Engage in physical assessments and patient triaging as necessary, employing the latest diagnostic tools
Document encounters and interventions meticulously within an electronic medical record system
Design, implement, and assess therapeutic plans, utilizing state-of-the-art, evidence-based practices
Manage medication therapy, including initiation and modification, within approved procedural guidelines
Order and interpret laboratory tests to monitor and enhance medication efficacy and safety
Report and document adverse drug events meticulously, adhering to best practices and regulatory standards
Provide comprehensive drug information and education, promoting informed decision-making among patients and providers
Mentor pharmacy students and residents, fostering the next generation of healthcare innovators
Adapt quickly to evolving healthcare demands, contributing to a culture of continuous improvement
Qualification
Required
PharmD or equivalent experience from an accredited pharmacy school
Completion of an ASHP-accredited PGY-1 Pharmacy Residency
Active license from the Texas Board of Pharmacy
Certification in Basic Life Support, First Aid, and CPR
Proficiency in computer applications, including Windows, Microsoft Word, and Excel
Ability to manage multiple priorities and deadlines effectively
Excellent communication and customer service skills, with a strong sense of professional discretion
Preferred
Board Certification as a Pharmacotherapy Specialist or Ambulatory Care Pharmacist
At least 2 years of community pharmacy and medication adherence counseling experience preferred
Experience in specialty areas such as HIV/AIDS, geriatrics, or pediatrics is advantageous
In-depth knowledge of advanced pharmacy practices and evidence-based medicine
