Responsibilities

Facilitates the development, implementation and evaluation of program services and initiatives

Plans, implements, evaluates, and coordinates appropriate patient care

Maintains program documentation and review to support regulatory and compliance regulations

Assists with determining appropriate training for the staff and developing the learning program

Collaborates with advanced practice providers to ensure quality outcomes

Collaborates with others to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures

May participate in injury prevention initiatives and/or activities

Works in a constant state of alertness and safe manner

Performs other duties as assigned

Registered Nurse (RN)Basic Life Support (BLS)Peritoneal DialysisProgram CoordinationPatient Care Coordination

Required

Bachelor's degree in Nursing or equivalent combination of education and experience

Three years' registered nurse experience

Basic Life Support Healthcare Provider (BLS HCP) required within 90 day of employment

Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)

Registered Nurse (RN) - Missouri Division of Professional Registration

Registered Nurse (RN) Issued by Compact State

Registered Nurse (RN) - Oklahoma Board of Nursing (OBN)

Registered Nurse (RN) - Wisconsin Department of Safety and Professional Services

Preferred

Prior Peritoneal Dialysis experience
