Responsibilities

Supervise the provision of participant’s health, behavioral, safety, social and medical support services
Oversee the development and implementation of participant IISPs reflecting the participant’s preference and concurrence
Monitor IISP goal progress and review documentation regularly; Provide IISP reviews or updates as assigned or needed
Review and assist in the implementation of Positive Behavior Support as needed or assigned; Make recommendations to the Health Care Supervisor and/or Clinical Manager for revisions
Provide direction and oversight to participant outings and activities
Facilitate participants accessing their community regularly for activities of their choice
Provide on-call and after-hour supervision, direction and support as needed or assigned
Coordinate, facilitate and assist with participant’s medical and behavioral programs
Maintain regular and consistent communication with participants medical/behavioral health team
Coordinate and assist with the provision of health care, nurse delegation and medication management; Schedule, facilitate or participate in training as needed or assigned
Consistently and accurately maintain client records in compliance with local, state, federal, agency and contractual regulations or requirements
Review and audit records periodically; Records include but are not limited to: consents, property inventories, leases, ISP’s, IISP’s, guardianship papers, nurse delegation paperwork and any other client record information
Review written and electronic documentation regularly; Address documentation errors promptly
Provide assistance and training on the use of objective, professional language and terminology as necessary
Assist with the supervision and management of both client’s individual and house financial accounts; Coordinate with appropriate personnel to protect participants’ financial resources
Supervise and oversee the staffing support in the homes, working to promote staff cohesion and build staff morale
Develop and post a staffing schedule that meets participant staffing needs
Monitor use of staffing hours against established guidelines; Notify supervisor of discrepancies
Promptly notify Human Resources of shift vacancy, leaves of absence or other adverse staffing situations
Provide on-site training to employees in accordance with state, agency and contract requirements
Provide personnel supervision to assigned staff
Annual reviews
Time card compliance
Performance management
Provide daily supervision of staff
Coordinate and facilitate monthly team meetings
Monitor and review staff responsibilities to ensure that the home(s) and agency vehicles are properly maintained and meet safety guidelines
Maintain and sustain positive working relationships with participants’ families and friends, vocational program staff, day program staff, DDA Case/Resource Management and other service providers
Participate in staff meetings and attend other meetings and seminars; complete individual training requirements annually and maintain certifications and licensures, as required
Attend mandatory training to complete:
First Aid / CPR
Blood borne pathogens
Mandated reporter
Continuing Education requirements for Long-term Care Workers
Operate a personal and/or agency vehicle on a regular basis; Maintain personal vehicle in a safe operating condition:
Maintain certifications and licenses, as a condition of employment:
Washington State Driver’s license and automobile registration
Automobile insurance that meets Washington State requirements
Provide driver’s abstract every year or upon request from agency
Perform other duties as needed or assigned on a daily basis

Required

Compile, categorize, calculate, audit, or verify information or data
Basic knowledge of software commonly used to perform the essential functions of the position (Word, Excel, Outlook, etc.)
Intermediate knowledge of principles and processes for providing supported living and personal care supports
Intermediate ability to handle complaints, settle disputes, and resolve grievances and conflicts, or otherwise negotiate with others
Able to encourage others and build mutual trust, respect, and cooperation among team members
Identify the developmental, educational or training needs of others; assist in the development of training programs or classes; facilitate teaching or instructing others
Basic knowledge of the principles of performance management and the supervision of personnel
Demonstrated ability to apply general rules to specific problems to produce answers that make sense and to combine pieces of information to form general rules or conclusions (includes finding a relationship among seemingly unrelated events)
Depending on individual client needs, this position may work with adaptive medical equipment (walker, wheel chair, Hoyer lift, wheel chair lift, gait belt, etc.) and may be required to provide hands-on or stand-by assistance
This position will operate an automobile to perform the essential functions of this position, therefore the use of vision and hearing are required for safety
Travel between work sites will be required
This position also requires the physical ability to support clients that have intellectual and developmental disabilities and may be assaultive or aggressive, medically fragile and/or unable to ambulate
Work sites may have flights of stairs and require the ability to traverse
Adaptive equipment may be used as long as physical safety is ensured at all times
