Responsibilities
Evaluates patients and makes decisions for Inpatient Rehabilitation admission as appropriate
Communicates with patients, families, physicians, acute care staff, Inpatient Rehabilitation staff, other team members and insurance representatives as necessary throughout the process
Maintain collaborative relationship with physiatry consult service, Inpatient Rehabilitation staff and other team members
Educate patients, families, health care providers, insurance representatives and community about Rehabilitation Center Programs
Qualification
Required
State of Vermont Registered Nurse (RN) license required
3-5 years work in rehabilitation program or with patients who need rehabilitation
Preferred
CRRN preferred
