Social Worker-Homemaker Home Health Aide (Program Coordinator)/Detail Temporary Promotion

Responsibilities
Coordinating the work responsibilities of clinical and support staff assigned to the program
Ensuring that the employees understand and adhere to the program directives, policies and standards as well as VAMC employee requirements
Assisting with establishing and maintaining the yearly H/HHA budget, as well as assisting in balancing the fund control point
Overseeing that payments to the H/HHA agencies are done in a timely manner and in compliance with VA standards
Ensuring that the Qualities measure that are prescribed in the VHA Handbook 1140.6 are completed and prepared for reporting to the Executive Leadership Team
Maintaining up to date files for the various homemaker agencies
Functioning as an active member of the CAVHCS GEC Committee
Tracking pending referrals, adding appropriate veterans, and for establishing and maintaining an Electronic Waiting list when budget resources are not sufficient to meet all identified needs
Participating in the Extended Care Leadership Committee, which includes presenting reports, action plans, budgets, and information
Overseeing the placement of veterans into the H/HHA program and assuring that the placements meet VA eligibility requirements
Providing appropriate documentation, reviewing and follow-up of complaints regarding the H/HHA program
Organizing and preparing reports concerning the HHA program for the Medical Center Director, VISN liaison and Central Office
Providing both informal and formal education to VA staff on any changes in VA policy concerning Homemaker/Home Health Aide Program
Attending and or participating in a variety of monthly and quarterly conference calls disseminating needed information to VA staff and community agency staff as warranted
Performing other related duties as assigned

Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE)
Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level
English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j
Physical Requirements: This position is mostly sedentary and requires walking, kneeling, bending, standing, and light lifting
One year of experience equivalent to the GS-11 grade level
Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency
Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience
Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program
Ability to supervise multidisciplinary staff assigned to the program
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s)
Ability to provide training, orientation, and guidance within clinical practice
