Responsibilities

Receives referrals for patients at high risk for neurobehavioral continued care planning needs from the admissions team and in-house. Ensures that all inappropriate referrals are channeled correctly and documented

Assesses patient needs and determines the mode of intervention. Interviews patients and/or families and records psychosocial assessments in accordance with policy, as documented in the chart

Implements the plan of intervention preparatory to discharge in compliance with departmental and governmental regulations

Involves the patient/family in the treatment planning process as demonstrated through chart notes and signatures on the appropriate hospital forms

Executes the plan of discharge/continued care, which is mutually agreeable to the patient/family

Monitors and evaluates operations for compliance with licensure, accreditation, and regulatory standards

Supports and facilitates staff professional development and participation in research programs

Demonstrates leadership and influences others in both the internal and external environments to promote optimal care for individual patients and optimal health care services for the public
