Responsibilities
Coordinate care and services for an assigned group of patients, with particular focus on individuals requiring additional support
Manage specialty referrals and follow-up to ensure timely access to care
Educate patients and families on care plans, self-care strategies, and available resources
Connect patients with relevant community programs, social services, and support organizations
Collaborate with external providers and agencies to support seamless transitions across care settings
Serve as a liaison between patients, providers, and care team members to promote clear communication
Support population health and care coordination initiatives within the clinic setting
Qualification
Required
Associate degree in a healthcare-related field
Combination of relevant education/training and experience, including at least one year in an outpatient care environment
Preferred
Bachelor's degree in a healthcare-related discipline
Prior experience in care coordination or care management
Background working in outpatient or ambulatory care settings
