Mid LevelEnsign Services is seeking an experienced PRN Remote Therapy Documentation Auditor to support therapy documentation review across multiple skilled nursing facilities within their KEYSTONE Strong market. This role involves conducting remote audits, ensuring compliance, and providing feedback to enhance documentation quality.

Responsibilities
Conduct remote audits of therapy documentation across multiple facilities
Review documentation for accuracy, compliance, and clinical appropriateness
Identify trends, risks, and opportunities for improvement
Provide clear, constructive feedback to therapy leadership
Support facility teams in maintaining regulatory readiness and documentation excellence
Collaborate with market and facility therapy leadership as needed
Audit therapy evaluations, daily notes, progress notes, and discharge summaries
Ensure compliance with Medicare guidelines, payer requirements, and Ensign standards
Monitor documentation consistency across disciplines and facilities
Assist with focused audits, education support, and documentation improvement initiatives
Maintain confidentiality and professionalism in all reviews
Qualification
Required
Current, active therapy license (OT, COTA, PT, PTA, or SLP)
SNF experience required
Strong knowledge of Medicare documentation requirements
Excellent attention to detail and written communication skills
Ability to work independently in a remote environment
Preferred
Experience with EMR systems (NetHealth, PCC, or similar preferred)
Prior auditing, DOR, ADOR, or leadership experience preferred but not required
