Responsibilities

Supervise, coach, and support Home Care staff (PCAs/Home Care Aides) to deliver safe, respectful, high-quality in-home care aligned with PACE standards, participant rights, and individualized care plans—providing real-time field support that reinforces dignity, safety, and service excellence
Manage daily operations including workload assignments, scheduling/coverage, attendance and call-outs, and timely completion of authorized services across participants’ homes to ensure continuity of care and minimize disruptions
Provide LVN-level clinical oversight for home care services, including in-home assessments within scope (vitals, symptom checks, basic clinical observations, functional/safety snapshots), documentation of findings, and timely escalation of changes in condition or safety concerns to RN/Provider/IDT per protocol
Conduct regular check-ins, field observations/ride-alongs, and competency validations; provide clear, consistent feedback and coaching, and reinforce infection prevention, safe body mechanics, and appropriate PPE use
Support hiring, onboarding, training, and ongoing development; lead performance management activities including recognition, corrective action, and development planning to promote engagement and retention
Collaborate closely with the IDT to implement care plans and support transitions (hospital/SNF/home); participate in IDT meetings as needed, provide updates on service delivery and observed needs/risks, ensure staff follow participant-specific instructions (ADL/IADL, transfers/mobility, safety precautions), and escalate concerns promptly (falls, functional decline, unsafe living conditions, suspected abuse/neglect, caregiver strain) per policy
Ensure services comply with DHCS/CMS PACE requirements and internal policies, including confidentiality/HIPAA; oversee visit documentation and timekeeping (EVV/visit verification, service notes, incident reporting) for accuracy and timeliness
Investigate, document, and resolve incidents, service failures, or complaints; implement corrective actions, track follow-through, and maintain positive relationships with participants and caregivers to support a consistently high-quality experience
During staffing shortages (PTO/call-outs/urgent gaps), provide direct participant care and/or complete critical visits within LVN scope to prevent missed services and maintain participant safety
Identify trends and operational risks (repeat call-outs, missed visits, overtime drivers, participant dissatisfaction); track and report key metrics (visit completion, timeliness, feedback, incidents, productivity) and partner with Operations/Clinical leaders on continuous improvement, workflow standardization, and program growth
Maintain professionalism, cultural humility, and respectful communication in all participant homes and team interactions; perform other duties as assigned to support participant care, program operations, and regulatory compliance
Take on additional duties as needed to support participant care, strengthen daily operations, and ensure ongoing regulatory compliance

Required

High school diploma or equivalent required
Active, unrestricted LVN license in the applicable state (CA, if applicable) required; maintains licensure without restrictions
2+ years of experience in home care, community-based care, personal care services, or a similar setting required
Experience supporting older adults and/or medically complex populations required
Demonstrated ability to support basic assessments (vitals, symptom observation), recognize changes in condition, escalate appropriately, and support care transitions in partnership with the clinical team
Working knowledge of ADLs/IADLs, safe transfers and mobility support, fall prevention, dementia/behavioral health considerations, infection prevention, and participant rights
Experience coaching and training staff, managing schedules/coverage, and leading performance management through clear feedback, documentation, and corrective action when needed
Strong collaboration skills with the ability to partner with RN/Provider/IDT, support care coordination, and de-escalate concerns with participants, caregivers, and team members
Highly organized with the ability to maintain accurate records (EVV/visit verification/timekeeping, service notes, incident reporting), follow policies/workflows, and ensure timely, complete documentation
Proficiency with EMR and scheduling/timekeeping systems, plus email and Slack/Teams; able to learn new platforms quickly and use basic reporting tools
Ability to handle sensitive information in alignment with HIPAA and organizational standards
Current CPR/First Aid certification required or ability to obtain within the required timeframe (per policy)
Ability to meet the physical requirements of the role, including frequent standing/walking, occasional lifting, and supporting safe mobility practices in participant homes
Ability to move freely and independently throughout the PACE center and in participants' homes and community settings, including navigating stairs, narrow walkways, uneven surfaces, and varying home environments
Ability to stand and walk for extended periods during in-home visits, center rounding, ride-alongs, and community travel
Ability to bend, kneel, reach, squat, and twist routinely while providing hands-on support, completing assessments, setting up supplies, and assisting participants in home or center environments
Ability to lift, carry, push, and pull up to 50 pounds (equipment, supplies, mobility devices) and assist with repositioning tasks, with or without reasonable accommodation
Ability to provide hands-on personal care support as needed, which may include assisting with bathing, toileting, dressing, grooming, and hygiene, and changing participants in accordance with care plans, infection prevention practices, and participant dignity
Ability to support safe transfers and mobility, including assisting participants to move between bed/chair/wheelchair/toilet/shower using proper body mechanics and mobility devices as appropriate
Ability to use hands and fingers for frequent grasping and fine motor tasks (documentation, phone use, PPE use, operating basic medical equipment, preparing supplies)
Ability to communicate clearly in person and by phone/video in environments that may be noisy or fast-paced, and to respond appropriately during urgent or emergent situations
Ability to drive locally between the center, participant homes, and community locations as required for field operations (valid driver's license and reliable transportation required)
Ability to perform essential functions of the role with or without reasonable accommodation, while consistently following safety, infection prevention, and participant rights standards

Preferred

Associate's or Bachelor's degree in healthcare, human services, business, or a related field preferred
Prior lead/supervisor experience strongly preferred
PACE or other value-based/interdisciplinary care model experience preferred
Bilingual English/Spanish preferred (or other languages based on participant population)
