Clinical Liaison - Registered Dietitian

Premier Infusion and Healthcare Services, Inc. is a preferred post-acute care partner providing home health and infusion services in Southern CA. The Clinical Liaison Registered Dietitian will interface with clinicians to facilitate home infusion referrals, assist in discharge planning, and work collaboratively with management to promote services and improve patient care.

Responsibilities

Partner with the Chief Strategy Officer and Territory Manager to create and execute account business and growth plan. Assist the Chief Strategy Officer in maintaining and growing relationships with referral sources
Communicate frequently with Chief Strategy Officer and the Territory Manager to discuss opportunities, assess progress, and provide feedback related to promoting the services of Premier Infusion and Healthcare Services
Work with Territory Manager and Chief Strategy Officer to develop long and short-range marketing goals and objectives
Provide the Management Team with intelligence from the field and recommend changes to effect improvement or react to evolutions in the industry and/or particular account
Conduct assessments of patients selected by the hospital for possible home infusion care and who could be referred to Premier Infusion and Healthcare Services
Educate patients, their families and the facility staff about the services and products provided by Premier Infusion and Healthcare Services
Collect, review and complete preadmission information and secure related data needed to assist the Intake Department. Effectively communicate with agency staff, medical team, patients and family throughout the discharge process to implement an effective treatment plan
Proactively initiate discharge planning with referral sources to ensure seamless patient transitions to home. Collect any data collection required for patient tracking process
Assist with determinations regarding appropriate IV administration method for patients referred to Premier Infusion and Healthcare Services, including all factors such as patient’s age, financial status, and social issues, etc
Review the patient’s medical record to obtain primary and secondary information insurance payer information and communicate with the Intake Department
Maintain confidentiality of patient and proprietary information and observes legal guidelines for safeguarding the confidentiality of patient and proprietary Premier Infusion and Healthcare Services information
If nursing license permits, conduct patient bedside hook-ups for specific therapies prior to discharge home
Provide visual bedside teaching to patients on all IV methods of administration when appropriate
Appropriately represent Premier Infusion and Healthcare Services to referral sources, physicians and patients
Participate in surveys conducted by authorized inspection agencies
Participate in the pharmacy’s Performance Improvement program as requested by the Performance Improvement Coordinator
Participate in pharmacy committees when requested
Participate in in-service education programs provided by the pharmacy
Pursue continuing education programs appropriate to job responsibilities
Report any misconduct, suspicious or unethical activities to the Compliance Officer
Perform other duties as assigned by supervisor
Comply with and adhere to the standards of this role as required by ACHC, Board of Pharmacy, Board of Nursing, Home Health Guidelines (Title 22), Medicare, Infusion Nurses Society, NHIA and other regulatory agencies, as applicable

Required

High School diploma or G.E.D or higher
Clinician with a License (Registered Dietitian)
Minimum of two (2) years' experience in home infusion services or satisfactory combination of education and experience
Prior experience with managed care contracts and marketing techniques
Strong working knowledge of reimbursement criteria for third party, government and commercial payers
Strong leadership skills with the ability to work across organizational lines
Professional and cooperative with a good aptitude for problem solving
Organizational skills sufficient to maintain consistently accurate records
Ability to evaluate options and to make efficient decisions
Computer skills that include proficiency with spreadsheets, word processing, and efficient use of the internet and e-mail
Excellent oral and written communication skills, with the ability to express technical issues in 'layman' terms

Preferred

Bachelor's Degree in Business or related field preferred
Two years of experience in the field
