Responsibilities
Lead and mentor a team of direct care staff and nurses
Coordinate and implement individualized care plans
Facilitate Interdisciplinary Team (IDT) meetings
Ensure compliance with DHS licensing and ICF regulations
Foster meaningful relationships with families, guardians, and case managers
Support individuals in setting and achieving personal goals
Manage household budgets and individual finances
Oversee program documentation, audits, and incident reporting
Provide hands-on care (20 hours/week), including 2 weekend shifts/month and 1 evening shift/week
Qualification
Required
Bachelor's in human services + 1 year direct care experience
Associate degree + 2 years direct care experience
Diploma in a related field + 3 years direct care experience
50 hours of training + 4 years of supervised direct care experience
Valid MN driver's license and clean driving record
Ability to obtain CPR/First Aid and Medication Administration certifications
Pass a DHS background check
Physical ability to lift 60 lbs. and support daily tasks
Caregiving: 2 years (Required)
Supervising: 1 year (Required)
Preferred
1 year of supervisory experience preferred
High school or equivalent (Preferred)
