Clinical Pharmacy Specialist (Community Based Outpatient Clinic)

Responsibilities
Maintains an outstanding and up-to-date clinical knowledge base regarding medication therapy, contemporary journal articles, and contemporary pharmacy practice
Provides support to leadership and management oversight as it relates to the clinical programs and clinical activities of assigned personnel
Determines opportunities to enhance pharmacy services to support patient education, patient adherence, clinical outcomes, and team member growth and development
Monitors drug utilization (formulary, non-formulary, restricted) and intervene to ensure formulary adherence and optimal evidence-based clinical outcomes
Monitors the educational needs of providers, pharmacists, and patients, and make recommendations to address any deficiencies. Actively participate in supporting any defined educational programs and activities
Analyzes and interprets the published literature, and appropriately apply one's findings. In addition, demonstrate a willingness to maintain a current understanding in terms of the breadth and depth of the literature in appropriate clinical areas
Participates in the development of quality and performance monitoring programs to support the optimal use of our personnel and medication resources
Implements and enforces departmental and/or institutional programs to support the wise use of available resources
Identifies and addresses the routine and the most salient needs of our all internal and external team members (patients, providers, and colleagues)
Acts as a liaison between service line leadership, pharmacy management, medical staff and healthcare personnel, and pharmacy team members
Supports the development, implementation, management, and monitoring of clinical and operational guidelines - national, VISN and local
Demonstrates the ability, willingness, knowledge, and wisdom to know with whom to communicate and when to resolve clinical and operational concerns

Citizenship: Citizen of the United States
Education: Applicants must possess one of the following: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree
Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia
GS-13 Clinical Pharmacy Specialist: Applicants must have 1 year of experience equivalent to the next lower grade level
Ability to communicate orally and in writing to persuade and influence clinical and management decisions
Expert understanding of regulatory and quality standards for their program area
Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise
Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy
Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters
