Serve as a client advocate for seniors and family members
Onboard new clients, guiding them through services and care expectations
Build and maintain strong client relationships
Support and monitor care plans and evolving service needs
Communicate client concerns to caregivers and care teams
Follow up to ensure client satisfaction and continuity of care
Document client interactions in care management systems
Qualification
Required
Experience in healthcare, home care, senior care, or client services
Strong communication, empathy, and problem-solving skills
Ability to manage multiple clients in a fast-paced environment
Customer service backgrounds also welcomed
