Responsibilities

Supervise and mentor RN Designees, RNs, LPNs, and home health staff

Provide clinical guidance for complex cases and emergencies

Complete initial patient assessments and develop individualized care plans

Utilizing the support of the RN Designee visit team facilitate and/or conduct supervisory visits to make sure care is safe, effective, and family-centered

Work closely with physicians, therapists, schools, and families

Partner with our intake and scheduling teams for smooth transitions of care

Participate in hiring, onboarding, and developing new nursing staff

Promote best practices in infection control, medication safety, and patient education

Qualification

RN license in NY2+ years nursing experienceTrach/vent experiencePatient assessmentsCare plans developmentLeadership experienceInfection controlMedication safetyPatient educationMentoringProblem-solving

Required

RN license in NY (required)

2+ years of nursing experience (home care or pediatrics a plus)

Trach/vent experience- or a strong interest in learning

A natural leader who enjoys teaching, mentoring, and problem-solving

Willingness to travel locally for supervisory visits with flexibility to do virtual visits after 6 (six) months of employment

Able to travel to Westchester, Orange, and Rockland County

Preferred

Leadership experience is a plus, but not required
