Centria Healthcare is a leading provider of home-based catastrophic injury and rehabilitation care, actively seeking compassionate Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) for a private duty nursing role. The position involves providing personalized care to a pediatric patient with complex medical needs, emphasizing safety and effective communication.

Responsibilities

Provide skilled nursing care, including but not limited to tracheostomy care, ventilator management (if applicable), catheter care, wound care, medication administration, and bowel/bladder programs
Assist with activities of daily living (ADLs) such as repositioning, transfers, and hygiene
Monitor the patient’s vital signs, symptoms, and overall well-being, reporting any concerns to the healthcare team
Offer emotional support and companionship, ensuring a positive and comfortable environment
Collaborate with family members, caregivers, and medical professionals to maintain continuity of care

Qualification

Required

Licensed RN or LPN in the state of Michigan
Ability to provide compassionate and patient-centered care
Strong communication and teamwork skills

Preferred

Experience in home health, private duty nursing, or long-term care preferred
Experience with tracheotomy and ventilator care (for pediatric positions)
Knowledge of paraplegia-related care, including skin integrity, mobility assistance, and respiratory support, is a plus
