Directing and coordinating the care provided by a multi-disciplinary team of home health care professionals

Processing incoming referrals

Management and mentoring of Clinical Associates and field staff

Facilitate new hire onboarding and orientation of field staff

Documentation review, including OASIS and Home Care Home Base (HCHB)

Monitoring quality assurance and process improvement

Managing staff productivity, workflow management and overall case management

Qualification

Required

An active Registered Nurse license in MO

Recent Medicare Certified Home Health Care experience

Comprehensive knowledge of the Medicare Conditions of Participation and regulatory compliance

Thorough understanding of OASIS documentation and ICD-10 coding

Must have current or previous management experience

Excellent organizational, interpersonal and communication skills

Preferred

Home Health experience preferred

HCHB a plus

Strong people management and leadership experience highly desired

Acute, med/surge, and/or sub-acute experience preferred/required
