Centria Healthcare is a leading provider of home-based catastrophic injury and rehabilitation care. They are seeking a dedicated Registered Nurse (RN) to provide intensive home health care for a pediatric patient, requiring strong assessment skills and experience in managing complex medical needs.

Responsibilities

Provide skilled nursing care (trach/vent care, medication administration, catheter care, etc.)

Monitor and document vitals, symptoms, and changes in condition

Assist with ADLs such as repositioning, hygiene, mobility, etc

Support patient well-being with compassionate and attentive care

Collaborate with families, caregivers, and the healthcare team

Qualification

Active RN licensePediatric assessment skillsHome health experienceCommunication skillsPatient-centered care

Required

Active RN or LPN license in the state of Michigan

Strong communication skills and a passion for patient-centered care

Preferred

Experience in home health, private duty, or long-term care preferred

Pediatric or adult care experience a plus (trach/vent knowledge helpful but not required)
