Responsibilities
Leads care teams using the Patient Centered Medical Home model, performing exams, assessments, and developing treatment plans for patients of all ages
Delivers care via in-person, telehealth, or phone, ensuring documentation meets legal standards, ordering tests, and adjusting treatments based on evidence-based practices
Supports patient access, education, and health promotion, manages acute and chronic conditions, and participates in after-hours call rotations
Ensures compliance with regulatory, funding, and clinical guidelines, maintains records, and participates in quality improvement and administrative tasks
Facilitates effective communication among staff, outside agencies, and patients, while practicing cost-effective medicine and documentation for billing
Qualification
Required
