The Clinical Pharmacist – SRT will be responsible for health promotion and disease prevention, including immunization administration and collaborative drug therapy management, across multiple locations in Florida.

Responsibilities

Provide individualized health promotion & disease prevention, including vaccine, long-acting HIV medicine administration, Diabetes monitoring, Nicotine reduction programs and other subject matters, in collaboration with other AHF Providers
Enhance the reputation of AHF by improving the outcomes of AHF patients, enhancing current AHF services, expanding opportunities for professional growth and satisfaction, and assisting with ongoing development of the AHF Healthcare System
Work as a part of a close-knit team to provide individualized health promotion and disease prevention, including the administration of immunization, Diabetes and Smoking Cessation programs, as well as other subject matter expertise (SME) programs developed by AHF, with appropriate collaborative drug therapy management authority
Includes prescriptive authority where this is legally & organizationally authorized, such as PrEP and PEP therapies
