Community Connections Specialist

Thrive Services is dedicated to supporting individuals in achieving independence through compassionate care. The Community Connections Specialist will be responsible for matching customers with appropriate employees for in-home care services, managing schedules, and ensuring effective communication between customers and staff.

Human Resources

Responsibilities

Manages schedule and phone calls as needed
Verifies and updates current Customers’ contact information
Notifies the employee of rescheduled and canceled services and updates the schedule as needed
Assesses the match between customers with the appropriate employee who can provide them with professional, compassionate, individualized in-home care
Conduct the Abuse and Incident reporting processes when necessary, and keep the Leadership Informed
Act as a liaison between the Customer and the Employee
Provide timely responses to Customer, Employee, or Staff requests
Execute the Customer’s and employees’ monthly follow-up calls
Coordinate the Meet and Greet visits, notate and execute the matches, and schedule as needed
Execute the reminders phone call for the meet and greets
Intervene, stabilize, and manage acute crisis situations and keep the Leadership Informed
Handles all return calls/voicemails or SMS regarding the Connections department
Register activities in the Customer’s profile

Required

At Least 18 Years of Age

Preferred

Associate Degree/Bachelors Degree preferred
Encouraged to have 40 WPM typing speed
Customer service: 1 year (Preferred)
