Responsibilities

Serve as the primary point of contact for patients requiring assistance navigating the healthcare system

Coordinate patient appointments, follow-ups, and referrals to ensure timely care

Assist patients with understanding treatment plans, medical instructions, and available resources

Assist providers with administrative tasks such as facilitating patient visits and timely documentation in patient charts

Identify and address potential barriers to care such as transportation, financial concerns, or language barriers

Provide education on insurance coverage, copays, and authorizations, working with billing teams as needed

Collaborate with providers, nurses, case managers, and other staff to ensure coordinated patient care

Track patient progress and follow up on missed appointments, ensuring continuity of care

Document all patient interactions, follow-ups, and outcomes accurately in the Electronic Health Record (EHR)
