Qualification
Required
Associate's Degree in a healthcare-related field
2 years of outpatient healthcare experience
Relevant coursework/training plus 1 year of outpatient experience
Preferred
Bachelor's Degree in a healthcare-related field
2+ years of care coordination experience
Experience working in an outpatient clinical setting
