Behavioral Health Clinical Team Lead

Central Maine Healthcare is seeking a Behavioral Health Clinical Team Lead to oversee the day-to-day management of the Integrated Behavioral Health Department at the Family Medicine Residency program. The role involves clinical supervision of clinicians, managing workflows, and providing direct patient care in individual and group settings.

Responsibilities

Clinical supervision of full-time clinicians and trainees when appropriate
Providing support and direction to the clinical and clerical staff regarding behavioral health workflows and scheduling needs
Managing the referral queue, and being a primary point of contact for other system departments
Coordinate the care management activities across the Opioid Health Home, ensure that there is a current Plan of Care/ITP for each member, and ensure that there is appropriate supervision of the Recovery Coach
The Behavioral Health Clinical Team Lead shall spend 50% of their time providing clinical services to the patient population of Family Medicine Residency to include individual and group psychotherapy and substance use treatment
Evaluates referrals to Behavioral Health for appropriateness for services and provides recommendations for alternative referral sources if the patient is not a good candidate for in-house services
Utilizes a Brief Intervention Model of care delivery
For patients enrolled in Behavioral Health services, conducts appropriate holistic assessments of needs and continuous reassessment of progress towards goals, using objective metrics
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors
Maintains current knowledge of resources available within the community
Communicates with outpatient physician regarding social service needs of the patient
Maintains supply of resource materials to be distributed to patients when needed
Is able to obtain other resources as needed
Communicates with family members and caretakers regarding the needs of the patient and anticipated plans as appropriate
Treats patients and their families with respect and dignity
Identifies and addresses psychosocial needs of patients and family
Interacts professionally with patient/family and involves patient/family in the formation of the plan of care
Accurately determines type of assistance/setting/resources necessary for the patient/family and provides appropriate resources/assistance/list of facilities
Provide counseling related to opioid dependency and individual or group substance use disorder outpatient therapy for OHH members
Provide behavioral health expertise and contributes to care planning, assessment of individual care needs, and identification of and connection to behavioral health services for all OHH members
Partner with Clinic and System Leadership related to operations and workflow support of the Behavioral Health team (e.g., workflow optimization, scheduling needs, ordering supplies, team member timecards, etc.)
Partner with Clinic and System Leadership related to financial oversight and management of grant-funded Behavioral Health and Addiction Medicine programmatic activities (e.g., tracking outcomes)
Partner with Clinical Leadership and System-based Behavioral Health Leadership to identify, monitor, and optimize Key Performance Indicators designed to improve Behavioral Health quality (e.g., visit volume targets, denials management, etc.)
Communicates appropriately and clearly to physicians, staff and administrative team
Demonstrates an ability to be flexible, organized and function under stressful situations
Maintains a good working relationship with the outpatient clinic staff and the community
Interacts with all individuals in a consistent manner, providing attention, support, and assistance to foster an environment of exceptional personal service
Contributes to a positive learning and teaching environment for students and residents by participating formal educational activities as requested, providing feedback in the context of clinical care, and being open for questions and consultation
Manages and operates equipment safely and correctly
Maintains regulatory requirements
Complies with all organizational policies regarding ethical business practices
Maintains confidentiality of information at all times
Maintains a safe work environment and performs duties of the position in a manner consistent with ensuring the safety of self and others

Required

LCSW, Licensed Master Social Worker – Conditional Clinical (LMSW-CC), LCPC, Licensed Clinical Professional Counselor – Conditional (LCPC-C), or Licensed Marriage and Family Therapist (LMFT) or Licensed Marriage and Family Therapist – Conditional (LMFT-C)
Completed a minimum of sixty (60) hours of alcohol and drug education within the last five (5) years
Who, within a maximum of five (5) years of initiating service delivery for OHH services, has completed sixty (60) hours of alcohol and drug education
Ability to engage patients and team members utilizing the CMH Experience Standards
Demonstrated ability to direct and triage in a highly fluid dynamic operational environment
Ability to collaborate with all layers of the management/ administration team

Preferred

Additional licensure as an LADC is preferred
Additional languages preferred
