Responsibilities
Ensure the health and safety needs of clients are met through assessment, supervision and education
Develop and implement appropriate service plans and plans of care, monitor client services, supervise home care staff and provide on-going training of home care staff including, determining competence prior to delegating tasks
Provide skilled nursing care to clients
Qualification
Required
Registered Nurse (RN) in the State of Minnesota
Driver's License
nurse client care coordination experience
Ensure the health and safety needs of clients are met through assessment, supervision and education
Develop and implement appropriate service plans and plans of care
Monitor client services
Supervise home care staff
Provide on-going training of home care staff including determining competence prior to delegating tasks
Provide skilled nursing care to clients
Ensure home care services are in compliance with current regulations and standards
Ability to relate professionally with clients, staff and family members
