Career Resource Manager

Adirondack Health Institute (AHI) is an independent, non-profit organization that is leading the way in rural health care transformation across a nine-county region of the North Country. The Career Resource Manager will lead the Healthcare and Education and Life Skills Program (HELP) grant, focusing on improving access to healthcare education and training for underserved communities while providing one-on-one case management to support students in overcoming barriers to their success.

Responsibilities

Provide one-on-one student resource case management and wrap-around support services to assist students with academic, personal, and financial challenges
Familiarize selected participants with program supports and requirements
Develop personalized plans for students, connecting them with appropriate resources
Develop and maintain rapport with program participants; meeting with participants on a regular basis to assess progress and to address and help resolve academic, financial, and personal barriers
Facilitate structured work-based learning opportunities to enhance participants’ practical skills and increase employment rates in healthcare roles
Monitor and support students’ progress, addressing challenges proactively
Coordinate with existing services/support programs to meet the needs of each student and develop individual goal plans focused on educational and/or employment goals with identified specific targeted outcomes
Monitor student progress and actively assist participants in setting and achieving education and career plans through individual interaction
Follow policies and procedures to verify participant eligibility and for the disbursement of financial supportive services
Track financial disbursements and balances for each participant
Develop and manage robust data collection and reporting systems to measure program impact, ensuring accurate and timely submission of required data
Utilize OSOS data collection system to manage, monitor, and track project progress and to report program outcomes
Develop positive working relationships and rapport with colleagues at SUNY Adirondack and faculty to support students’ academic success
Establish and maintain strong relationships with community-based organizations to connect students to additional resources such as housing, healthcare, and mentorship
Document and communicate program progress, challenges, and outcomes to SUNY Program Director
Assist with program evaluation and reporting by coordinating data collection and evaluation methods and preparing and assisting with program reports as required

Required

Bachelor's degree in social work, healthcare, or education-related field required
Minimum of two (2) years' experience in case management or experience providing services to students required
Demonstrated experience and understanding of case management standards and best practices required
Experience working with underrepresented, rural, and economically disadvantaged groups required
Ability to work remotely, however, must have the ability to travel as required
Must have a valid Driver's License and vehicle, or evidence of mobility within service area
Must be able to write and speak clearly using the English language to convey information
Must be able to hear at normal speaking levels in both person and over the telephone
Must be able to walk or stand
Must be able to use fingers and hands to feel objects
Must be able to remain in a stationary position for extended periods
Specific vision abilities required by this job include close vision, peripheral vision, depth perception and the ability to adjust focus

Preferred

Demonstrated experience and understanding of healthcare trends and occupations highly preferred
Knowledge of community resources, such as those who support medical, behavioral, and social needs, highly preferred
