They are seeking a High-Risk Breast Program Coordinator to serve as a primary contact for patients, coordinating care and collaborating with providers to implement effective program components and improve quality services.

Responsibilities

Serves as primary contact and resource to navigate patients throughout the breast service line and if applicable, the multidisciplinary high-risk breast cancer program
Coordinates and facilitates the plan of care for risk assessment and prevention
In collaboration with providers, develops and implements program components, assessment tools and protocols to create comprehensive and seamless service delivery through the multidisciplinary provision of care to patients
Serves as point of contact and subject matter expert to collaborate with referring providers, supplying information and assistance as needed to promote quality services and accomplish organizational goals
Assists in developing relevant policies and procedures
Participates in quality improvement initiatives and activities for performance, process, and quality
Establishes annual program goals in collaboration with providers and hospital leadership. This includes development, implementation, and evaluation of strategies to achieve annual goals
Meets with Practice Manager and physician/hospital leadership to monitor program development
Respects and maintains confidentiality
Works in a safe, compliant and ethical manner at all times

Qualification

Wisconsin RNLPN licensureClinical knowledge of breast surgeryCPR certificationProgram/project developmentEducational tool developmentComputer competenciesMultidisciplinary collaboration

Required

Current Wisconsin licensure as Registered Nurse or Licensed Practical Nurse required
Clinical knowledge of breast surgery, operating room, or post-surgical experience necessary
CPR certification required
Experience in program/project development and the ability to successfully execute program initiatives
Ability to develop educational tools in support of program and patients
Demonstrated ability to work closely with physicians, patients, clinical staff, management, and administration
Computer competencies with applicable site software are necessary

Preferred

Previous experience working with multidisciplinary process for breast patients preferred
