1+ years expParadigm Health is an exciting Indiana-owned company that brings a new 'Paradigm' to Home Care. They are looking for a compassionate and driven Hospice RN On Call to perform assessments, manage pain, and respond to the needs of terminally ill patients and their families.

Responsibilities
Performs physical and psychosocial assessment on all hospice patients
Ability to adequately assess and reassess pain
Utilizes appropriate pain management techniques
Educates the patient and family regarding pain management
Responds to the physical, mental, social and emotional needs of terminally ill patients and their families
Assesses patient’s condition, initiates plan of care, re-evaluates and updates as necessary
Obtains all necessary physician orders and collaborates with physician as patient’s needs dictate
Initiates case conferences to discuss multidisciplinary team responsibilities, pain management, bereavement counseling, etc
Qualification
Required
1 year of RN experience required
Licensed Registered Nurse in State of Indiana
Current driver's license
Must have reliable transportation and insurance
Preferred
One year Hospice experience preferred
