Medical Assistant Care Coordinator

The Medical Assistant Care Coordinator will manage and coordinate patient care, ensuring safe transitions and facilitating communication between patients and healthcare providers.

Responsibilities
Receive and review procedural notes for cardiology patients (e.g., PCI, pacemaker/ICD implantation, ablations, heart failure admissions, valve procedures)
Contact patients within the defined timeframe (typically 24–72 hours post-discharge – ie. 2 business days for TCM) to conduct medication reconciliation, verify that prescriptions, anticoagulation plans, and medication changes are accurately implemented, execute post-discharge plan as per orders and discharge summaries, ensure that patients understand discharge instructions, and escalate clinical needs to appropriate clinical team members
Schedule and coordinate follow-up visits with cardiologists, advanced practice providers, and diagnostic services (e.g., echocardiograms, stress tests)
Collaborate with the Intensive Cardiac Rehabilitation team to facilitate enrollment of appropriate patients, or facilitate referral to traditional cardiac rehab when necessary
Support adherence to care plans by addressing barriers such as transportation, medication access, or understanding of instructions
Serve as a clinical resource to patients and families, offering guidance and triage as appropriate
Document patient interactions thoroughly within the electronic medical record (EMR) and escalate issues to the care team as needed
Assist with remote patient monitoring programs (e.g., heart failure weight checks, vital sign monitoring, device clinic communication)
Collaborate with cardiologists, APPs, nurses, and hospital transition-of-care teams to ensure continuity
Provide updates on high-risk patients and participate in interdisciplinary meetings or huddles
Act as a liaison between the cardiology practice and external providers, including rehabilitation programs, skilled nursing facilities, and home health agencies
Support quality improvement initiatives related to readmission reduction, patient satisfaction, and care transition workflows
Ensure compliance with all regulatory and documentation requirements
Qualification
Required
Certified Medical Assistant
Current BLS certification
Minimum 2 years of clinical cardiology experience
Minimum 2 years of transitional care management and/or care coordination of cardiology patients
Knowledge of cardiovascular disease, diagnoses, and medications
Excellent communication skills
Ability to interpret clinical documentation and cardiac terminology
Strong organizational skills with attention to detail
Proficiency with Athena EMR systems and care-management platforms
