They are seeking an experienced Hospice & Home Health Marketer / Community Liaison to build and maintain referral relationships with various healthcare providers and drive census growth.

Responsibilities
Develop and maintain strong referral relationships with hospitals, SNFs, ALFs, physicians, case managers, discharge planners, and community partners
Conduct in-service presentations and educational events on hospice and home health services
Identify new business opportunities and strategic partnerships
Maintain consistent presence in the community through face-to-face visits and outreach
Track and report referral activity, market trends, and growth metrics
Collaborate closely with intake, admissions, clinical leadership, and executive team
Ensure all marketing activities comply with Medicare, state, and ethical guidelines
Qualification
