Educating patients about vaccinations
Outreach activities, including going to farmers markets, social media
Introducing mental health resources for those who are interested
Providing patients newly diagnosed with diabetes with the clinic’s easy-to-read introductory booklet of dietary and physical activity recommendations
Attending staff and/or team meetings
Qualification
Required
Fellows will be required to attend weekly Career Pathways Program meetings
