Responsibilities
Clinical Care Management: Conduct comprehensive patient assessments, develop individualized care plans, and coordinate multidisciplinary healthcare services
Patient Education: Provide expert guidance on disease management, medication compliance, and health maintenance strategies
Documentation Excellence: Utilize WellSky Kinnser EMR and Andy AI technology for accurate, timely clinical documentation and OASIS assessments
Care Coordination: Collaborate effectively with physicians, specialists, and healthcare team members to ensure seamless patient care
Quality Assurance: Monitor patient progress, adjust care plans as needed, and ensure adherence to clinical standards and regulatory requirements
Family Support: Educate and support family members and caregivers in patient care techniques and safety protocols
Qualification
Required
Valid Illinois Registered Nurse license
Current CPR certification
Valid driver's license and auto insurance
Open to new graduates and experienced nurses - comprehensive training provided
Willingness to learn WellSky Kinnser EMR system (training included)
Strong clinical assessment and critical thinking abilities
Outstanding written and verbal communication skills
Patient-centered care philosophy with cultural sensitivity
Strong organizational and time management capabilities
Passion for community service and home-based healthcare
Authorization to work in the United States or eligibility for H1B sponsorship
English proficiency equivalent to successful nursing practice in the U.S
