Responsibilities
Performing duties as outlined in the plan of care such as Medication Administration, Personal Care, Light Housekeeping and day to day support
2:2 Ratio of individuals through a consistent caseload
Effectively communicating updates and changes to patient's status to the supervising RN
Maintaining a safe patient environment
Providing emotional and social support and compassionate care to patients
Maintaining current patient records and progress notes
Working as a member of the interdisciplinary team
Developing and establishing a plan of care with physician
Qualification
Required
Minimum of an associate's degree or equivalent in Nursing from an accredited nursing program
RN license
Valid driver's license, car insurance and reliable transportation
Preferred
Nursing experience within Home Care/ Home Health, Private Duty Nursing, Long Term Care, or Disability Services is a plus but not required
