Responsibilities

Overall responsibility for managing and coordinating department activities

Ensures staff compliance with organizational policies and external regulatory agencies and takes leadership responsibility to coordinate the integration of the department's patient care and discharge planning processes with related hospital departments and external agencies to ensure continuity of care and optimal clinical resource utilization

Facility needs a candidate with extensive case management leadership experience

Self-motivated, high performer, strong work ethics, able to review data and develop action plans independently

Must have strong strategic relationships with physicians and post-acute community

Director will report to Director VP of CM

790 Beds (this unit covers the entire hospital)

Trauma Level II Facility

Over 34,000 annual admissions, nearly 88,000 ED visits, and over 215,000 outpatient visits

FTEs: roughly 110 - direct reports are 4 managers & 1 Social Worker Supervisor and an AA

Director oversees CM, Social Work, and the Chaplain Dept

The candidate may need to rotate on call hours for the weekends, depending on volume and what is going on in the hospital - this may vary
