Case Manager-Financial Services
Lifespan of Greater Rochester is a mission-driven nonprofit dedicated to supporting older adults. The Case Manager in Financial Services is responsible for assessing clients' financial needs, managing their financial affairs, and assisting them in achieving long-term financial security through various community resources.

Responsibilities
Assess and investigate financial situation of clients
Determine and plan appropriate actions that will assist client in becoming as financially secure as possible
Provide full bill paying and budgeting services to ensure long-term security
Assist with financial decision- making
Provide referrals to other community resources assisting the client to remain as independent as possible
Maintain and manage all records, case notes, billing, reports and metrics as required ensuring accuracy and compliance with agency departmental policies and procedures, including state and federal tax laws
Advocate with financial, creditor and other institutions and agencies on behalf of client
Appropriately bill clients for services rendered
Participate in all Lifespan staff meetings, trainings, continuous process improvement and in the implementation of agency mission and vison
Other duties as assigned by supervisor
Qualification
Required
Associate in applied sciences required
2 years' banking or financial services experience required
Must have strong financial acumen and a working knowledge of community resources
Must be able to meet with individuals in their home environment and in a variety of community settings including hospital and nursing home environments
Must be able to meet with clients present in their homes and have a valid NYS Driver's License
Ability to drive tactical results to achieve strategic goals
Must demonstrate Knowledge of the eligibility requirements and application processes of benefits and programs applicable to clients, including but not limited to SSI and Medicaid, MMLTC, VA, Medicare, SNT
Must have attention to detail and, ability to multi-task
Strong organizational, verbal, written and telephone communication skills required
Preferred
Bachelor's degree in business, Finance, Human Services or equivalent
Expertise in MEDICAID and other benefit progress
Prior experience working with and advocating for older adults
Computer experience preferred (Word, Excel, EMS, Office 365, web research)
