On Call Registered Nurse
Luminary Hospice is a mission-driven organization focused on providing compassionate end-of-life care. The On Call Registered Nurse is responsible for delivering nursing care during after-hours, conducting patient assessments, developing care plans, and coordinating patient care in collaboration with the hospice interdisciplinary group.

Responsibilities
Completes an initial and comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es)
Provides professional nursing care by utilizing all elements of nursing process and utilizes health assessment data to determine nursing diagnosis
Assesses and evaluates patient's status through writing and initiating plan of care, re-evaluating patient and family/caregiver needs after hours when needed, and participates in revising the plan of care as necessary
Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care
Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care
Supports the Case Managers and assumes responsibility to coordinate patient care for the agency after hours
Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers
Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver
Actively participates in quality assessment performance improvement teams and activities if needed
Perform additional job duties as assigned
Qualification
Required
Bachelor's or Associate's degree in nursing or related fields, required
Maintains an active nursing license that is in good standing in the state performing job duties
Maintains an active CPR certification
Knowledge of hospice care and the services provided to patient and family/caregiver through an interdisciplinary group
Proficient in MS Office applications and ability to learn department and job-specific software systems
Demonstrate organizational skills
Demonstrate effective verbal and written communication skills
Demonstrate analytical skills when problem-solving
Demonstrate high attention to detail and a high degree of accuracy
Preferred
At least 2-5 years of experience in the healthcare management, hospice, or palliative care and/or training in end-of-life care, preferred
At least 1 year of experience with Medicare Hospice Certification as well as CHAP guidelines, preferred
