Responsibilities
Coordinate patients’ activities to facilitate the appropriate delivery of health care services and transition of care across the continuum in the Emergency Department’s PH2S/HealthNow Program
Intervene with populations most at risk for threats to health and well-being at the earliest opportunity and provide linkage to care to improve health outcomes
Perform a wide range of clinical-support, recruitment of study participants and administrative functions to fully support the program
Interview patients to elicit health needs history; administer questionnaires in a courteous, professional, and empathetic manner; conduct point of care testing and coordinate testing for HIV and Hepatitis C; refer to appropriate community resources and provide linkage to care; maintain patients records ensuring strict confidentiality; maintain appropriate tracking and reporting systems; collect, analyze and Q/As data for inclusion in progress reports
Carry out successful screening, intervention and linkage to care of patients presenting to the Ohio State University Wexner Medical Center facilities for various health related services
Maintain communication with patients and coordinate linkage to care services post discharge
Implement day to day activities for the HealthNow, following-up with patients post discharge, and screen/enroll patients for various research studies
Receive referrals from the Health Promotion Advocates/review lab reports to determine if patients may be eligible for HealthNow program activities
Complete required release of information (ROIs), provide appropriate linkage to care, and develop/maintain on-going communication with community partners
Consent and enroll participants in eligible research studies
Address patient concerns through active listening and escalate as needed to the care team
Provide linkage to care for diseases of public health importance and ensure referrals are completed and patients’ needs are addressed
Research and remain up to date on available services/programs both within and outside of the OSU network that may benefit patients in order to successfully match patient needs with resources
Maintain appropriate tracking and reporting systems and complete other related duties as assigned
Employ motivational interviewing (MI), open ended questions and client centered counseling to collect patient information and ensure the necessary information is promptly passed on to the care team
Participate in the recruitment of study participants; collect and provide relevant data to be used in assessing eligibility of patients for participation in protocols; assist with administration of informed consent process; assist with cognitive testing both clinically and for research visits; assist principal investigator (PI) and sub-investigator with continuity of care of study patients during and between clinic visits; assist with collection, entering, maintenance and reporting of clinical research data; assist with analysis and interpretation of research data; enter protocol, medical history, and demographic information abstracted from patient charts into appropriate data capturing system; enter patient clinical data into electronic medical record system; review data and progress notes with principal investigator and sponsor agencies as appropriate; compose and respond to correspondence from principal investigators; prepare appropriate protocol files for data audits; assist with development of data collection plan and forms for new research protocols; prepare summaries of research results for protocol evaluations and dissemination activities
Qualification
Required
Bachelor's degree in public health or related field (or equivalent number of years of experience)
Community Health Worker (CHW) certification preferred but not required
