Responsibilities
Coordinate care with the member and their medical, health, substance use, mental health and other related providers to ensure integrated treatment and care
Provide and coordinate post hospital discharge follow-up, as needed
Maintain contact with payer systems communicating and documenting coordination activities and service authorizations
Provide initial and on-going member engagement, support and care coordination
Partner with the member to develop and implement a person-centered care plan
Connect members with community services and resources
Convene interdisciplinary care team and coordinate activities to ensure integration
Promote hope and empowerment through meetings and support with members
Facilitate access to transportation/ mobility resources that enable members to fully participate in medical services, behavioral health services and community resources
Ensure that clients are treated with dignity and respect in accordance with ECHS’ policy
Qualification
Required
Requires a Bachelors degree in Psychology, Social Work or related field
Experience working in care management or case management for adults
Valid Massachusetts drivers license
