Palliative Nurse Case Manager
Mission Hospice & Home Care serves patients and families in the San Francisco Peninsula and South Bay with quality care and compassionate end-of-life support and education. The Palliative Nurse Case Manager will work as a team member to provide skilled nursing assessment and care to patients and their families with chronic diseases.

Responsibilities
Assume primary responsibility for a patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals, and interventions
Coordinate all patient/family services and prioritize needs with the members of the interdisciplinary team (IDT)
Use a case management approach and refer to other services as needed
Coordinate the implementation of the plan of care and ensure care coordination for each patient
Provide palliative nursing evaluations to patients and their families including assessment, planning, goals, and interventions
Seek input from other team members regarding patient plan of care to obtain additional knowledge and support
Qualification
Required
One year experience as a professional nurse in the past three years or Bachelor's degree in Nursing from an accredited School of Nursing
Minimum of two years of professional nursing experience with at least two years in an acute care, home health or palliative care/hospice setting
Current license as a registered professional nurse in the State of California
Preferred
HPNA certification
