Responsibilities
Supports C3 ACO providers by encouraging members and caregivers to engage in care management
Helps develop outreach plans and connects individuals and families with needed resources
Serves as the primary contact for Community Partners, builds positive relationships, and makes referrals when appropriate
Builds trusting relationships and uses various outreach methods to engage members in different settings
Addresses cultural and language barriers while coaching members toward personal and clinical goals
Assists with scheduling, SDOH screenings, care coordination, and follow-up needs
Helps members access community services such as Medicaid, food assistance, housing, and transportation
Participates in care team meetings and maintains required documentation
Qualification
Required
Basic knowledge of Microsoft Office and ability to learn the health center's Electronic Medical Record and other applications
Excellent written and oral communication skills
Successful completion of the CHW core competency training within 2 years of employment
