They are seeking an experienced Registered Nurse responsible for providing direct patient care in the patient’s home and coordinating care.

Responsibilities
Providing direct patient care in the patient’s home
Conducting assessments and developing care plans
Working collaboratively with the patient’s physician to provide optimum care in the home
Coordinating care and identifying the need for other services in the home
Traveling/ floating throughout the south shore
Qualification
Required
Current Massachusetts Nursing license in good standing
2+ years of nursing experience required
Strong clinical assessment skills and the ability to prioritize and triage patients
Exceptional communication skills and the ability to work within a team structure
Excellent organizational skills and time management skills
Ability to work independently and understanding of how to access resources for patients
Strong computer skills
Preferred
Registered Nurse (bachelor's degree preferred)
