Responsibilities
Provide essential care coordination and logistical support to field staff
Oversee patient supply orders, tracking, and delivery so clinicians have the right tools at the right time
Monitor patient care and clinical oversight to ensure excellent outcomes for our homebound community
Perform triage and wellness calls, assisting with clinical follow-up as needed
Handle incoming clinical faxes and route them appropriately to keep care moving
Collaborate with the Scheduling team to meet patient care needs efficiently
Qualification
Required
Active California RN License (Required)
Minimum 2 Year Nursing Experience (Home Health experience preferred)
The 'Helper' DNA: A genuine desire to collaborate across departments and support fellow clinicians
Tech Proficiency: Comfortable with EMR systems and Apple devices
Highly Organized: Ability to multitask and prioritize in a high-volume office setting
Professional Communicator: Excellent phone manner and interpersonal skills
