Pennant Services is seeking an RN Case Manager to join their Home Health team in Goodlettsville, TN. The role involves managing and delivering the full plan of care for home health patients, ensuring high quality clinical services while maintaining effective communication with patients and healthcare professionals.

Responsibilities

Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes

Make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer

Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care

Establish and maintain an effective and efficient visit route

Provide high quality clinical services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, from admit through discharge

Qualification

RN LicenseClinical RN ExperienceCPR CertificationPatient CareDocumentation Skills

Required

One (1) year of clinical RN experience

Current RN license in the state where services are rendered

Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Must be able to reliably commute to multiple patient residences daily within the geographic area assigned
