They are seeking an experienced Home Health QA Nurse (LPN or RN) to ensure clinical accuracy, regulatory compliance, and maintain high standards of care through documentation review and support for field staff.

Responsibilities

Review clinical notes for completeness, accuracy, and compliance
Ensure documentation supports Home Health eligibility, visit frequencies, and medical necessity
Monitor timeliness of orders, signatures, and documentation submission
Support clinicians with OASIS accuracy and required corrections
Assist with compliance related to CMS, state regulations, and agency standards
Collaborate with the QA team to ensure high-quality and consistent documentation
Provide guidance to field staff on documentation requirements
Communicate documentation needs clearly and professionally
Assist with QAPI activities, tracking, audit prep, and quality improvement initiatives
Work closely with Clinical Supervisors, RN Case Managers, and office staff
Maintain excellent communication and follow-up for required documentation
Support smooth operations across Home Health services

Required

Active LPN or RN license in Oklahoma
Home Health experience required (QA, Case Management, OASIS, or field nursing)
Strong understanding of Home Health regulations, documentation standards, and CMS requirements

Strong communication skills and ability to partner with field clinicians
Organized, self-directed, and comfortable managing multiple deadlines
