Responsibilities

Reviewing their medication history with them at the bedside

Reach out to our patient’s pharmacy to review their lists of recently filled medications

Obtain their most up to date medication history

Review their medication lists for discrepancies

Have changes validated by pharmacists

Work closely with the attending providers offering feedback of major findings that may have been missed

Work in a dynamic environment with other interdepartmental disciplines like inpatient pharmacist, nurses, and case management/social work

Qualification

Pharmacy Technician LicenseCertified Pharmacy Technician2 years' experienceHigh school diploma

Required

High school diploma or equivalent

2 years'+ experience

Valid Pharmacy Technician License in state of practice

Certified Pharmacy Technician (CPhT)
