This role focuses on ensuring seamless transitions of care and supporting patient and family education while maintaining high standards of safety and compassion.

Responsibilities
Admit and discharge patients entering from community settings and coordinate intrahospital transfers
Complete documentation related to admission and discharge assessments and initial treatment plans
Assist units with patient arrivals, departures, therapeutic passes, and hospital discharges
Provide nursing care as needed, including medication administration, patient monitoring, and crisis response support
Participate in treatment team meetings and patient/family education
Communicate effectively with multidisciplinary teams to support safe, efficient, and therapeutic care
Maintain compliance with hospital policies, nursing standards, and the Montana Nurse Practice Act
Qualification
Required
Graduate of an accredited LPN program
Current LPN license in the State of Montana (or Nurse Licensure Compact state)
CPR and Nonviolent Crisis Intervention with Advanced Physical Skills (within 3 months of hire)
Preferred
1–2 years of experience in psychiatric or pediatric nursing
